PROGRAM DESCRIPTION:
The Asthma Alliance of North Carolina invites you to attend the annual North Carolina Asthma Summit. The 2008 Summit will feature the recently released Guidelines for the Diagnosis and Management of Asthma (Expert Review Panel 3), along with interpretation and related application. The Summit will also provide an excellent opportunity for asthma champions throughout North Carolina to showcase success stories and share strategies for dealing with asthma in our communities.

Participants will interact with supportive leaders within our state and other advocates who have made a significant impact on individuals with asthma. Afternoon breakout sessions will offer practical asthma-related information, resources, and exciting new projects.

Update! - There will also be two events surrounding the 2008 Asthma Summit. The first will be a "Breathe Well Live Well" training held by the American Lung Association on April 16th, 2008 at the Hawthorne Inn. The second event will be a "Asthma & Spirometry Workshop" on April 18th, 2008 at the Hawthorne Inn.

ASTHMA SUMMIT REGISTRATION INFORMATION:
Registration Fee:

$50 fee if registered by April 3, 2008. After April 3, 2008, the registration fee is $60.
All payments must be received before April 17th, 2008! You cannot make payments on-site. (You must be pre-registered in order to attend, You cannot register at the time of the event.)

Methods of Payment Accepted Online:
1. Credit Cards: Visa, MasterCard, Amex, Discover, Diners Club
2. Checks: You can register online and select "Check" as the payment method. HOWEVER, we do not actually process the check electronically. You DO NOT have to enter the bank name, check number, and check date when registering. This information is OPTIONAL. You will receive an email stating that the registration fee is "due by check".
After registering online, mail your personal or agency check made payable to: "American Lung Association of North Carolina" to the address listed below. Please included a copy of your registration receipt along with the check to avoid errors in processing.
Note about online registrations:
When registering multiple persons online, you must do so one at a time.
